YUCAIPA ADULT DAY CENTER

ADULT DAY HEALTH CARE FACILITY (ADHC) LOCATED IN YUCAIPA, CALIFORNIA

The Yucaipa Adult Day Center is located at 12980 2nd Street, Yucaipa, California.    The center is a licensed, community-based, day program of health care, therapies and social services for the frail elderly and younger physically or mentally impaired adults and is designed to restore or maintain the capacity for self care to the fullest extent possible and to prevent unnecessary institutionalizations.

The Yucaipa Adult Day Center opened April of  1994.

We are licensed by the Department of Health Services and monitored by the Department of Health Services and the Department of Aging.  Our center’s capacity is 49.  Our programs include:

  •  Nursing services with RN on site
  •  Socialization
  •  Recreational and Social Activities
  •  Meals and Nutritional Counseling
  •  Physical and occupational therapy
  •  Speech Therapy
  •  Psychiatric and Psychological Services
  •  Transportation to and from the Center

Our core staff includes:

  •  Administrator
  • Social Worker
  • Registered Nurse
  • Activity Coordinator
  • Program Aides

Participant Eligibility:

The Adult Day Health Center is for frail elderly and younger functionally impaired adults with-

  1. A medical condition requiring treatment or rehabilitative service prescribed by a physician, and

 

  1. Physical/mental impairments that handicap activities of daily living but are not so serious as to require 24-hour institutional care, and

 

  1. Reasonable expectation that preventative service will maintain or improve the present level of functioning, and

 

  1. A high potential for further deterioration and probable institutionalization if ADHC were not available.

 

All four of these conditions must exist before ADHC services can be authorized.
