PLACEMENT AND ADMISSIONS PROCESS

Below is a brief summary of the admission process into our facilities.
ICF/DD-H OR ICF/DD-N PLACEMENTS
The JonBec Care facility’s shall admit ambulatory and non-ambulatory developmentally disabled clients to our ICF/DD-H and ICF/DD-N facilities based on eligibility for services and availability of open beds.
Clients shall be admitted to the facility without regard to the religion, ethnic origin, sex, degree of disability, or ability to pay. The facility shall accept and retain only those clients for whom it can provide specific programming and care. All clients shall have had a comprehensive evaluation covering physical, emotional, social, and cognitive factors conducted by an appropriately constituted Interdisciplinary team. If a client is admitted for whom there is an inappropriate program, the facility shall state this in the client’s record and initiate plans for continued and active exploration of alternatives. In the event of the emergency admission of a client, a comprehensive evaluation of the client shall be completed within thirty (30) days of admission.
Procedure
Referral from Regional Center: The Regional Center is the placement agency for our facilities. When a potential client or his/her family is interested in admission, they shall notify the regional center and the regional center will refer the client to our facility for possible admission. Pre-Admission Screening: Appropriate JonBec staff will conduct a preliminary evaluation based on the placement packet of information submitted from the regional center. The preliminary information must contain background information as well as currently valid assessments of functional developmental, behavioral, social, health and nutritional status to determine if the facility can provide for the client’s needs and if the client is likely to benefit from placement in one of our facility’s.
Clients eligible for admission to the facility shall meet current admission criteria (based on Federal and State regulatory requirements) as follows:
1. Require intensive programming (active treatment) in one (1) of the following skill areas:
2. Self-Help Program: an eligible client is one who has no or minimal self-care skills and requires assistance in carrying out all activities of daily living, or has some self-care skills but requires monitoring or assistance and individualized training to maintain or improve self-care skills. Programming for clients requiring assistance in self-care skills shall include training in at least two (2) of the following:
-Self-feeding
-Habit training
-Bowel and Bladder control
-Dressing
-Grooming and Hygiene
3. Nursing Service: an eligible client is one who requires intermittent nursing supervision by the RN consultant.
4. Behavior Shaping
5. Socialization Training
Face to face visits: Once the facility has determined that the client is eligible for admission, the facility shall schedule an Admission meeting to evaluate the referral information, meet the potential client and family, and give the potential client and family a tour of the facility.
The Admission Committee comprised of the Administrator, Executive Director, QMRP, Nurse, Regional Center case manager, parents, client (wherever possible) and other members of the ID team will confer and determine whether or not the placement is appropriate.
At the time of the Admission meeting, the committee ascertains whether or not the potential client is a Medi-Cal recipient. If he/she is not, the parents or authorized representative are requested to obtain the necessary forms for the potential client to receive Medi-Cal funding. If the parents/authorized representative does not wish the client to become a Medi-Cal recipient the client will be considered a “private pay” client and the parent/authorized representative will be responsible for paying all expenses incurred by the client.
Admission Scheduled: If everyone is in agreement of the placement based on the Admission Meeting and visits, a date will be scheduled for the client to be admitted to the facility.

ADULT DAY HEALTH CARE PLACEMENTS
Participant Eligibility:
The Adult Day Health Center is for elderly and physically or mentally impaired adults with-
- A medical condition requiring treatment or rehabilitative service prescribed by a physician, and
- Physical/mental impairments that handicap activities of daily living but are not so serious as to require 24-hour institutional care, and
- Reasonable expectation that preventative service will maintain or improve the present level of functioning, and
- A high potential for further deterioration and probable institutionalization if ADHC were not available.
All four of these conditions must exist before ADHC services can be authorized.
